SUMMARY OF BENEFITS

The following represents an overview of the benefits package you are entitled to as an employee of The Dutra Group:

HEALTH INSURANCE

The employee and their immediate family (children up to age 26) are eligible to participate in Group Health Insurance. Coverage takes effect on the first day of the month following 30 days of employment. Dutra offers plans through Anthem Blue Cross and Kaiser Permanente.

 

Employees who reside in California can choose from any of the plans listed on the Medical Authorization/Declination Form. Employees residing outside of California can choose from one of the two (2) Anthem PPO plans offered. Dutra pays 75% of the employee’s premium.

DENTAL INSURANCE

The employee and their immediate family are also eligible to participate in the Group Dental Insurance. Coverage takes effect the first day of the month following 30 days of employment. This program is provided by Anthem Blue Cross. It is a PPO plan which provides a Network of dentists you can go to so you can get the most coverage. Please note this includes Orthodontia benefits!

VISION INSURANCE

The employee and their immediate family are also eligible to participate in the Group Vision Insurance. Coverage takes effect the first day of the month following 30 days of employment. This program is provided by Anthem Blue Cross.

LIFE INSURANCE AND AD&D

Along with the health care coverage shown above, the company provides life insurance coverage through Anthem Blue Cross in the amount of one times your annual salary up to a maximum of $50,000. Accidental Death and Dismemberment coverage up to $50,000 is also provided for the employee.

LONG-TERM DISABILITY

Also provided, at no cost to you, is a Long-Term Disability plan through Anthem Blue Cross. If you are disabled, the plan pays 60% of your salary up to a maximum of $10,000 per month.

EMPLOYEE ASSISTANCE PROGRAM

This program is provided to you at no cost through the company. It covers you and your family members. The purpose of the EAP is to help you prevent or solve personal problems that occur in your life. You have access too free, confidential, professional services to help you deal with problems. Also available are financial and legal assistance.

PRE-PAID LEGAL SERVICES

This voluntary program is provided to you through monthly payroll deductions. It covers legal expenses (see enclosed information) for everyone in your home. Like life insurance or car insurance, this legal protection covers you anywhere in the United States whether you are at work or on vacation. Participation is on a month-to-month basis and the cost can never be increased for the current level of coverage.

CREDIT UNION

Employees have available to them The Redwood Credit Union which offer their members checking and savings accounts, VISA/MC, auto loans, secured loans, life insurance, and additional member-only benefits.

401(K)

Employees are eligible to participate in the 401(K) plan after 3 months of employment. Unless you specify otherwise, you will be automatically enrolled at the rate of 4% on the first of the month following 3 months of employment. The Dutra Group 401(K) Plan has been established through John Hancock. The company matches 50% ($0.50 for each dollar) of your contribution up to 8% of your gross income. As with all 401(K) plans, you are always 100% vested in the employee contributions. For the company-matching portion, employees vest at a rate of 20% per year. When you terminate, reach age 60 or become totally disabled, you will have access to your contributions as well as the appropriate vested company-matching amount.

INCENTIVE BONUS PLAN

An incentive bonus plan is provided to employees if two company criteria have been met: (1) if operational budgets are met and (2) there are excess earnings. The potential for individual employees can be from 0% to 15% of base salary.

VACATION AND SICK LEAVE

Employees are entitled to two weeks (ten working days) of paid annual vacation. After five years, the employee is eligible for 3 weeks of paid annual vacation. After fifteen years, the employee is eligible for 4 weeks of paid annual vacation. Employees are entitled to nine (9) days of paid sick leave each year.

HOLIDAYS

The company observes eight paid holidays as scheduled through the calendar year.
