BENEFITS
As a member of the Dutra family, you are eligible for the following
benefits packages:
HEALTH INSURANCE
The employee and their immediate family (children to age 18 or, if
full-time students, age 24) are eligible to participate in the Group
Health Insurance. Coverage takes effect the first of the month following
30 days of employment. Dutra's health care provider is Blue Cross of
California. Options include Blue Cross HMO (California Care) and Blue
Cross PPO (Prudent Buyer). MetLife provides dental coverage with a $1,500
maximum benefit annually. Vision coverage is provided through VSP.
LIFE INSURANCE AND AD&D
Along with the health care coverage shown above, the company provides life
insurance coverage through Blue Cross of California in the amount of one
times your annual salary up to a maximum of $50,000. Also, Accidental
Death and Dismemberment (AD&D) coverage up to $50,000 is provided by the
company.
EMPLOYEE ASSISTANCE PROGRAM
This program is provided to you at no cost through the company. It covers
you and your family members. The purpose of the EAP is to help you prevent
or solve personal problems that occur in your life. You have access to
free, confidential, professional services to help you deal with problems.
Also available are financial and legal assistance.
PRE-PAID LEGAL SERVICES
This voluntary program is provided to you through payroll deductions for
$15.95 a month. It covers legal expenses for everyone in your home. Like
life insurance or car insurance, this legal protection covers you anywhere
in the United States whether you are at work or on vacation. Participation
is on a month-to-month basis and the cost can never be increased for the
current level of coverage.
BAY CITIES CREDIT UNION
Bay Cities Credit Union offers its members checking and savings accounts,
VISA/MC, auto loans, secured loans, life insurance, and additional
member-only benefits.
401(K)
Employees are eligible to participate in the 401(K) plan after 3 months of
employment. You can enroll on the first of any month after meeting the
3-month eligibility. This 401(K) Plan has been established through ING.
The company matches 50% ($0.50 for each dollar) of your contribution up to
7% of your gross income. As with all 401(K) plans, you are always 100%
vested in the employee contributions. For the company-matching portion,
employees vest at a rate of 20% per year. When you terminate, reach age 60
or become totally disabled, you will have access to your contributions as
well as the appropriate vested company-matching amount.
INCENTIVE BONUS PLAN
An incentive bonus plan is provided to employees if two company criteria
have been met: (1) if operational budgets are met and (2) there are excess
earnings. The potential for individual employees can be from 0% to 25% of
base salary.
VACATION AND SICK LEAVE
Employees are entitled to two weeks (ten working days) of paid annual
vacation. Vacation may be taken as it is accrued and must be taken in the
year for which it is accrued. After five years, the employee is eligible
for 3 weeks of paid annual vacation. After 90 days of employment,
employees receive 5 days of sick leave per calendar year. Neither vacation
nor sick leave may be carried over from year to year.
HOLIDAYS
The company observes eight paid holidays as scheduled through the calendar
year.
